When to send a CoTreat Pack
Send a pack when:
- A new treatment is recommended
- A patient needs time to think
- You want them to review options at home
- There are visuals (X-rays, scans, photos) that help explain

When not needed
You may not need to send a pack if:
- It’s a routine follow-up with no new information
